Meadow Hall School (MHS) may wish to take photographs or make video recordings of your child for our website or promotional purposes. These images may appear in our printed publications, on DVD, on our website, or all three.

As a matter of policy, MHS has designed this form to obtain your permission before we publish any images of your child from which he/she may be identified. Please answer questions 1 to 4 below, before signing and dating the form.

Please note that websites can be viewed throughout the world, not just in Nigeria where Nigerian law applies.

Conditions of use of images of pupils:

  1. This form is valid throughout your child’s time at Meadow Hall School, unless you inform the school otherwise in writing.
  2. MHS will not include details or full names (which means first name and surname) of any child or adult with an image on DVD, on our website, or in printed publications, without reason.
  3. MHS will not include personal e-mail or postal addresses, or telephone or fax numbers on DVD, or on our website.
  4. MHS will only use images of pupils who are modestly dressed, to reduce the risk of such images being used inappropriately.
